Right near the top of the list, just below the virus//worm writers are 
the people who blithely open up any attachment sent to them.  They are 
the same ones who don't have/run/update AV programs.  Personally, I use 
a Mac and two firewalls (hardware and software) and two AV programs.  
The wife's Windows box is behind the hardware firewall, has a software 
firewall and and AV program.
